I (we) intend or am about to engage in certain activities which have been organized by the Flatirons Ski Club, Inc., (hereafter called “Club”).

I (we) am doing so entirely on my own initiative and responsibility, and I am fully aware that risks and hazards are, or may be, inherent in such activities. I elect to voluntarily engage in such activities understanding and accepting that it is not possible to describe all of the risks associated with these activities.

I (we) further understand and expect that my conduct during such activities shall be dictated by my own decisions as to, if, and how to deal with any particular set of conditions.

I (we) understand that the Club, its officers, directors and trip leaders in particular and its members in general organize such activities solely for the pleasure and enjoyment of the participants.

I (we) am supplying for my own use such first aid, emergency communications, safety and medical supplies as I deem necessary, and I am not relying upon the Club and its officers, directors, leaders or members for such items.

I (we) recognize that neither the Club nor its several officers, directors, leaders or members is responsible for my participation in any particular activity nor the participation of other people and under no circumstances is the Club or its officers, directors, leaders or members responsible for my conduct or the conduct of others sharing such participation.

My (our) participation is in no way based upon any representations, statements or comments made by any member, officer, director or leader as to the difficulty, danger or hazard of such activities, but rather because of my independent desire to participate in such activities with other people having similar interests.

Therefore, for myself, my heirs, personal representative, assigns, and other interested parties, I hereby release and forever discharge the Club and all of its officers, directors, agents, leaders, and members, individually and collectively, when acting in the capacity of an official or representative of the Club, from any and all claims or causes of action now or hereafter arising on account of my death or on account of any injury to me or damage to my property or on account of the death, injury or property damage suffered by any other person which may occur from any cause while I, or any other person be engaged in activities of the Club, including therein, but not limited to, all ground and other transportation operations incident thereto, unless death, injury or property damage result from willful misconduct.

This Waiver/Release shall be binding in all of its terms for all activities of the Club.

I hereby certify that I, we have read the foregoing Waiver/Release, understand it in all its terms and conditions and sign it voluntarily and intelligently, and have been offered a copy of it, this ____ day of ______, ________.
